The Quality Management System (QMS) software market hit an inflection point in 2024-2026. Every vendor — from the platform incumbents to the Y Combinator AI-native challengers — is shipping LLM features. The interesting question is no longer “do they have AI.” It is “which of the eight parts of the regulated-AI implementation pattern do they actually ship, versus claim, versus leave to the customer.”

We spent the week mapping this. Here is what we found.

The Tension: Non-Determinism vs. Validation

LLMs are non-deterministic. Regulated industries require validation, auditability, and reproducibility. That is the binding constraint. A CAPA narrative draft that produces slightly different wording on each invocation is a Category 5 GAMP problem unless the LLM is pinned, grounded, and kept out of the actual decision loop.

QMS vendors are bridging that gap with the same playbook. The variance is not whether they use AI — they all do — it is how defensibly they ship it.

The 2024-2026 Forcing Functions

Three regulatory currents are reshaping who wins the table:

  • FDA QMSR transition — 21 CFR Part 820 is being harmonized with ISO 13485:2016, effective in the 2026 window. Every med-device QMS must revalidate templates, training, and evidence. This is the single biggest sales tailwind for med-device specialists in 2026.
  • ISO 42001 (AIMS) — the AI Management System standard is becoming the audit floor. MasterControl is the first major QMS vendor certified (July 15, 2025). Dot Compliance has publicly claimed it. The vendors that cannot show ISO 42001 by 2027-2028 will look like SaaS vendors that could not show SOC 2 in 2018.
  • Veeva AI Agents GA (Dec 3, 2025) — Veeva shipped its first agents in CRM and PromoMats. Quality Event Agents are in development. The era of “agentic GxP” is no longer a roadmap slide.

The 8-Part Regulated-AI Pattern

After auditing 25+ QMS / EQMS vendors across Tier 1 platforms, Tier 2 pure-plays, and Tier 4 AI-native disruptors, the implementation pattern converges on eight parts. The same eight. Every time.

  1. Keep the LLM out of the decision loop. AI as co-pilot, not autopilot. Human review + e-signature before any GxP record becomes a record. MasterControl’s AI Trust Center says it most directly: “MasterControl’s AI features do not perform any decision making tasks.” Veeva’s Quality Event Agent “generates narrative summaries” — the human still writes the final investigation.

  2. Pin and lock the model like validated software. Model version + adapter + prompt-template + tool-set versioned in a model registry. Model changes are change-control events. “Automated evals” are regression test suites. Veeva’s JD for Senior Software Engineer – AI Applications literally calls for “automated validation… to ensure agent behavior remains consistent across model updates and feature releases.”

  3. Make outputs deterministic where you can, stochastic only where you must. Push the LLM into suggestion and retrieval. Keep template-based and rule-based systems for the actual GxP work — the slots, the citations, the structured forms. This is GAMP 5 Category 4 (configured) or Category 3 (non-configured, hosted validated software), not the un-validatable Category 5.

  4. Control the data, citations, and grounding. RAG over the customer’s own validated corpus: SOPs, deviations, CAPAs, complaints, batch records, training records, change controls. Two reasons: it keeps the answer grounded in the customer’s actual policies, and it makes the answer auditable — you can show which document, paragraph, and revision was used.

  5. Run on infrastructure you can defend. Three patterns, in increasing order of customer flexibility:

    • Own everything (MasterControl): “MasterControl does not utilize third party services to provide AI functionality… our secure agentic AI platform… a system of customized large language models (LLMs), services, and programmatic agents all strictly governed and administered by MasterControl.” This removes the foundation-model vendor from the customer’s audit scope. Real architectural moat.
    • Hyperscaler-regulated-perimeter (Veeva): “Veeva AI Agents use large language models (LLMs) from Anthropic and Amazon, hosted on Amazon Bedrock. Custom agents… use Veeva-hosted models or customer provided models hosted on Amazon Bedrock or Microsoft Azure AI Foundry.” Customer picks the LLM and the cloud.
    • Bring your own model (rare, growing): larger pharma wants to plug in their own fine-tuned internal model. Vendors offering “any LLM, any cloud” win the enterprise.
  6. Treat prompts, retrievers, and tool APIs as configuration items. Versioned, reviewed, e-signed. Retrieval indexes validated, refreshed under change control. Tool APIs allowlisted. Every agent invocation logged with input, output, model version, prompt version, retrieved documents, tool calls, and the human who approved it. This is where most vendors are weakest on the public artifact side.

  7. ISO 42001 (AIMS). MasterControl certified July 15, 2025. Dot Compliance has publicly claimed it. By 2027, every serious vendor will need this on the trust page.

  8. Human-in-the-loop as contractual, not aspirational. The “is this feature opt-in?” / “does the human approve before record?” questions are moving from marketing copy → contract clauses → explicit validation deliverables the customer hands to their auditor. MasterControl’s trust FAQ answers “Is usage of MasterControl Generative AI optional?” explicitly. That Q&A is the new category of artifact every serious vendor will produce by end of 2026.

Per-Vendor: Who Ships What

We sampled the public artifact set for the vendors that matter in 2026. Pattern: Tier 1 platforms own the trust narrative, Tier 2 pure-plays own the AI-first vocabulary, Tier 4 disruptors own the architectural bets.

Veeva (NYSE: VEEV) — Vault Quality Cloud / “QualityOne”

Ships 1, 2, 3, 4, 6, 8. Custom Agents for “any LLM, any cloud” maps to part 5. Veeva AI Agents GA Dec 3, 2025 across Vault CRM (Free Text, Voice, Pre-call) and PromoMats (Quick Check, Content). Quality Event Agents in development for Investigations and CAPA. The bet: model-agnostic, customer-owns-the-choice-of-LLM, runs on Vault Platform that already has Part 11 / GxP controls. Footprint is expanding: Kindeva adopted Veeva Quality Cloud on June 1, 2026.

MasterControl — Integrated QMS + MES + CMMS

Ships 1-8. AI Trust Center is the most explicit and regulator-friendly public commitment set in the field. Six AI features in 14 months: GxPAssist (Jul 2024), Document Summarizer (Feb 2025), Master Template Generator (May 2025), Regulatory Chat (Aug 19, 2025), SOP Analyzer (Jan 27, 2026), Event Summarizer (Apr 21, 2026). $200M ARR Sept 4, 2025. ISO 42001 certified July 15, 2025. FedRAMP authorized May 5, 2025. 1,100+ customers. Acquired Qualer March 3, 2025 — now the only vendor claiming integrated QMS + MES + CMMS on one platform. The architectural moat is owning the whole stack: agents, models, infrastructure. That removes the foundation-model vendor from the customer’s audit scope.

Qualio — The “Agentic Compliance” Re-positioning

Ships 1, 3, 4, 6, 8. Rebranded 2025-2026 as “The Agentic Compliance Platform for Life Sciences.” Compliance Intelligence is the agent product: gap analysis across FDA, ISO, and EMA standards, plus 28 frameworks, with cross-mapped evidence. Strong mid-market story. Part 5 is SaaS multi-tenant — less defensible than MasterControl’s owned stack, more defensible than a Salesforce substrate. The wedge: built specifically for the FDA QMSR transition and EU MDR.

Greenlight Guru — Med Device Specialist

Ships 1, 3, 4, 6, 8. Purpose-built for med device / IVD / SaMD. 1,100+ MedTech companies (matching MasterControl’s customer count claim). Their public wedge is the QMSR resource hub — the med-device regulatory transition is the entire 2026 marketing play. AI on top of an already-tight QMSR-shaped data model means better grounding, fewer hallucinations. Acquired by Epista Life Sciences in March 2022; product has stayed med-device-only.

Dot Compliance — AI-First + ISO 42001 Claimed

Ships 1, 3, 4, 6, 8, 7 (claimed). Salesforce substrate, AI-first positioning, QMSR-ready templates. Notably, Dot Compliance has publicly claimed ISO/IEC 42001 (AIMS) alongside ISO 9001, 27001, and 27017 — joining MasterControl in the “AI-first and AI-certified” lane. NA + Israel roots. The architectural bet is that Salesforce customers will consolidate quality onto the same substrate.

Honeywell (ETQ Reliance)

Ships 1, 3, 6, 8. Differentiator is process-industry IIoT data — temperature, pressure, batch, sensor streams — fed into QMS. “Should we open a deviation?” becomes answerable with sensor data. A pure-software QMS vendor does not have that. Acquired by Honeywell in September 2021, sits inside Honeywell Forge / Connected Enterprise.

Seal (seal.run) — The AI-Native Challenger to Watch

Ships 1, 3, 4, 6, 8. Y Combinator + SNR-backed. Durham NC + London. Seventeen-capability blueprint covering DHF / DMR / DHR, MDR / IVDR, batch record review, APR / PQR, stability, and environmental monitoring. GAMP 5 2nd edition + SOC 2 Type II. Tagline: “Your QMS is a post-mortem.” Founded ex-biopharma, going after biologics / med device / CDMOs. Gap: no public AI Trust Center yet. For a regulated buyer, that is the first thing to ask for.

ComplianceQuest, Qualityze, Scilife, Ideagen, SimplerQMS, ZenQMS, AssurX

All Tier 2 mid-market pure-plays. Generally ship 1, 3, 6, 8. Scilife differentiates on “Smart Quality” (gamification, augmented learning). Ideagen (Nottingham, 18,000+ customers) is the largest by user count and won G2 2026 Best UK Software for the third consecutive year. ComplianceQuest and Qualityze are Salesforce-substrate. None of them have an ISO 42001 certification we could verify as of this writing.

TrackWise (Sparta → Honeywell)

Ships 1, 3, 6, 8 (verified from public Sparta ASEAN page). Both on-prem and Salesforce-cloud editions. Still in the top-4 EQMS comparison set as of June 6, 2026 alongside MasterControl, Qualio, and Veeva QualityOne.

The Vendor Evaluation Checklist

If you are buying a QMS in 2026 and care about regulated AI, the 10 questions to ask are:

  1. Is the AI feature opt-in, or on by default? (Must be opt-in.)
  2. Is there a human-in-the-loop approval step before the AI output becomes a GxP record? (Must be.)
  3. Is the model pinned to a specific version, and is the model itself under change control? (Must be.)
  4. Can the customer bring their own model, or are they locked to the vendor’s? (Negotiable.)
  5. Is customer data used to train any model that other customers might hit? (Must not be.)
  6. Is the agent’s prompt + retrieval index + tool allowlist a validated configuration item? (Must be.)
  7. Is there an automated eval suite that runs before any model bump ships, with a defined pass/fail threshold? (Must be.)
  8. Is the LLM’s data flow documented end-to-end, with a Part 11 / GxP audit trail, including the human who approved each output? (Must be.)
  9. Does the vendor hold ISO 42001, and can they show an AIMS statement? (Strongly preferred.)
  10. Is the AI feature covered by the vendor’s validation package, or is re-validation the customer’s problem? (Strongly preferred: covered.)

If the vendor says “we use AI, it’s magic” to any of those questions, walk.

What is Still Genuinely Unsolved (Mid-2026)

  • LLM observability in a GxP way. LangSmith, Langfuse, Phoenix, Helicone — all built for engineers, not Part 11 auditors. Real GxP-grade eval + observability is a market without a clear winner.
  • Continuous learning in a GxP world. Effectively given up by most vendors. Pattern: train on customer data in a sandbox, validate the new model, ship the new pinned version. Same release cadence as a software update.
  • Multi-agent workflows and emergent behavior. Current architectures (planner, retriever, tool-caller, writer, human approver) work because each agent is bounded and tool-constrained. Agents calling agents, asynchronous events, self-correction, long-lived coordination — that is the open frontier. Validation gets harder the more agents you compose.
  • Regulators’ posture is still settling. FDA, EU AI Act, MHRA, PMDA, Health Canada all have AI-in-medical-products guidance, but the specifics of validating a continuously-deployed, retrieval-augmented, multi-agent system against Part 11 and Annex 11 are still being worked out in 2024-2026. Vendors that help customers navigate that conversation have a real trust moat.

The Open Questions for the Rest of 2026

  • Will Veeva’s “QualityOne” rebrand stick as the public face of the quality product, or stay an internal SKU? (We saw it surface in the IntuitionLabs June 6, 2026 comparison.)
  • Will Seal’s ambitious 17-capability scope (biologics + med device + CDMO + EM + stability + APR) hold up at scale, or will they narrow to a focused wedge?
  • Will the ISO 42001 race force Veeva, Qualio, Greenlight Guru, TrackWise, and Seal to certify by 2026-Q4 or 2027-Q2?
  • Will Complere’s SEO-driven comparison content convert, or get absorbed by MasterControl’s and Veeva’s content marketing?
  • Will ServiceNow’s GRC/QMS ambitions translate into a real regulated-life-sciences product, or stay as a checkbox?

The Bottom Line

LLMs in a Part 11 world are not magic. They are drafts, suggestions, summaries, and evidence. The 8-part pattern shows up because the regulatory regime forces it — keep the LLM out of the decision loop, pin the model, ground in customer data, own or constrain the supply chain, treat prompts and retrievers as configuration items, keep human-in-the-loop mandatory, ship an automated eval harness, certify against ISO 42001.

The vendors that have internalized the pattern are shipping. The vendors that have not are still selling “AI-powered” on the homepage.
